We do both on every project, so the site is designed and then genuinely built rather than assembled from a theme: custom layout, custom code, and whatever the business actually needs behind it, like online booking, e-commerce, a donation form or a calculator. If you have been told a custom feature is not possible on your current site, that is usually a platform limit rather than a real one.
